ST ANNE’S HOLIDAY DIALYSIS CENTRE - TERMS AND CONDITIONS
Accommodation: St Anne’s provides self-catering holiday accommodation for up to three families at a time. Each family will have their own private bedroom(s), lounge and kitchen. The sun lounge, swimming pool and garden are shared with other occupants of the house and non-residents using the dialysis facilities. There is a pay telephone for use in the hall.
What is provided: Towels and bedding are provided for the accommodation. Please bring your own towels for use at the swimming pool and tea towels for the kitchen. The kitchen is fully equipped with cutlery, crockery and cooking equipment. There is a full oven and a microwave. Each lounge has a TV. Hairdryers are provided in each bedroom.
Arrival and Departure: Please arrive in the afternoon of your appointed day between mid-day and 4 pm. You will be met and shown around by Mrs Veronica Cumming, the Resident House Manager. You are asked to vacate your bedrooms by 9 am on your last day, and to vacate the living room and kitchen by 10 am.
Dialysis: Our non-resident Renal Nurse, Sharon Cartwright will contact you the week before your holiday to finalise dialysis arrangements.
Haemodialysis: This takes place in the purpose built dialysis unit in the garden and accommodates 3 patients at any one time using B Braun Dialog + machines, patients must have dialysed for a minimum of 6 months prior to their holiday dialysis. Patients must bring their own Dialysers & fully labelled medication required whilst on dialysis.
CAPD: Your unit must arrange delivery of fluids to arrive the week before your holiday. Please check that this has been done before your arrival.
Medication: Please bring with you any medication you will require during your holiday which you are currently taking.
Pets: We regret that we cannot accommodate any pets on the premises
Valuables: The Trust cannot accept responsibility for any valuables and/or personal possessions.
Guests and Fire Safety: We must have the names of ALL persons who will be staying at St Anne’s (including children). The register must be signed on arrival. Only persons whose names are on the booking form may stay. There can be no substitutions of names without prior written notification.
Smoking: Smoking (including the use of E cigarettes) is not permitted anywhere in the house, the dialysis unit or the swimming pool changing room.
St Anne’s Holiday Dialysis Centre is available FREE OF CHARGE to dialysis patients of the West London Renal and Transplant Centre, their immediate next of kin or partner (one carer) and their own children 12 years of age & under.
The following charges apply to extended family members, transplant patients, elder children and patients from other hospitals and from other countries, who are of course most welcome at St Anne’s. Dialysis patients from WLHHDT will have priority.
We are unable to accept patients who are HIV, Hepatitis B or C positive, assessed by the referring unit as medically unstable during dialysis or have been dialysing less than 6 months

Booking Fee: A booking fee of £25 payable to West London Hospitals Holiday Dialysis Trust is required with this booking form. It will be returned to you on arrival at St Anne’s or if there is a medical reason for cancellation. For unreasonable cancellations this will be withheld.
Provisional Bookings can be made by calling 01243 376514 (Monday-Friday, 9am-2pm and answerphone) They will be confirmed when a completed booking form and £25 deposit are received. You must check your confirmation details carefully as soon as you receive them and advise Sharon Cartwright immediately if anything appears incorrect, as later amendments may not be possible. If you have not received confirmation within 14 days please contact St Anne’s.
Payment (if applicable) is due at least 4 weeks before your arrival. This should be made payable to West London Hospitals Holiday Dialysis Trust and sent c/o Sharon Cartwright, St Anne’s, 34 Havant Rd, Emsworth, PO10 7JG.
Cancellation: If it should become necessary to cancel your holiday this must be confirmed in writing by the person who signed the booking form or your Consultant if there is a medical reason.
Any holiday taken at St Anne’s & treatment provided is subject to the jurisdiction of English Law.
The Trust reserves the right, at its absolute discretion, to cancel or curtail holidays or refuse admission to the property.
